The following forms are available to CVAD Faculty and Staff. Submitted forms will be sent to CVAD IT Support and the appropriate departments.
Terms of Agreement
Current faculty, staff, and students. The CVAD Student Computer (SCL) Equipment Checkout service current faculty, staff, and students to check out state-owned equipment for university use (academic or professional) for a set amount of time, free of charge. Review the terms on the Equipment Checkout Terms of Agreement carefully - by using this service, you signify your agreement to these Terms of Agreement.
Faculty Planning Form
Current CVAD faculty and instructors have exclusive use of the fabrication labs and makerspaces during the first twelve (12) class days of the long semesters. This time can be used for training, class preparation, and research project planning. In preparation for using the labs for class assignments (to use the lab equipment as part of their course), instructors need to complete the Fabrication Labs Faculty Planning Form (webform) at least a week prior to the beginning of the semester.
Invoice for CVAD Fabrication Lab Access
If you are currently enrolled in a CVAD course, you are not required to pay any additional fees to use the equipment in the CVAD Fabrication Lab. If you are not enrolled in a CVAD course you are required to purchase the CVAD FabLab pass.
Safety Training Form
In order to make a reservation to use machines and equipment in the CVAD Fabrication Lab (digital or textile), you must first complete safety training. Once you have completed this training, submit your information on the CVAD Fabrication Lab Safety Training Form (webform).
CVAD Faculty Enhanced Privileges Agreement
Current faculty. The College of Visual Arts and Design recognizes that the research responsibilities of its faculty require that elevated privileges be granted to them on their personally assigned computer(s) and\or grant-funded equipment. After submitting the CVAD Faculty Enhanced Privileges Agreement (pdf) form, your information will be reviewed and consideration will be given.
Permission to Use Student's Work Form
Current Faculty, Staff, and Students - use when photographing or video recording student work. The CVAD Photo Documentation Service is open to CVAD students, faculty, and staff. Part of the function of the service is to create a database of student work that the university can pull from when creating promotional materials. Because of this, you will be required to sign a Permission to Use Student's Work (pdf) release form in order to have your work photographed. Artists that are not associated with the college or not currently enrolled in a CVAD course can submit the Permission to Use Artist's Work (pdf) release form.
